Screening Tools 

AUDIT (PDF 23KB) - The World Health Organisation’s Alcohol Use Disorders Identification Test (AUDIT) is a very reliable and simple screening tool which is sensitive to early detection of risky and high risk (or hazardous and harmful) drinking. It has three questions on alcohol consumption (1 to 3), three questions on drinking behaviour and dependence (4 to 6) and four questions on the consequences or problems related to drinking (7 to 10). 

DASS 21 (PDF 8KB) - The Depression, Anxiety & Stress Scale 21 is a short version of the DASS42. The tool includes 21 questions which assess symptoms of depression, anxiety and stress in adults. Download the DASS Scoring Template and DASS Scores to assist in using this tool. 

K10 Outcome Tool (PDF) - The K10 is widely recommended as a simple measure of psychological distress and a measure of outcomes following treatment for common mental health disorders. The K10 is promoted on the Clinical Research Unit for Anxiety and Depression website as a self report measure to identify need for treatment and is a suitable measure for people with anxiety and depressive disorders.
